Position Purpose

Advocate for designated clients. Participate as a team member in the delivery of case management and treatment services to clients and their families and facilitate the issues of recovery into their daily living situations. Provide continuity of program services in an ethical, legal and moral manner within a safe and therapeutic environment, consistent with applicable regulatory and accreditation standards. Promote the treatment concepts and work cooperatively with all staff to implement treatment services.

Job Requirements

  • Bachelor's Degree in Psychology, Social Work, or other related field.
  • One year human services or case management experience; two years (preferred).
  • Valid driver's license.

Qualifications

  • Demonstrated education, training or experience.
  • Adequate written skills to accurately complete required documentation within the time frames prescribed.
  • Ability to learn and work independently to complete documentation in software systems.
  • Ability to present and coordinate lectures for clients, parents, staff or community resources.
  • Skilled in behavior management techniques.

Essential Responsibilities

  • Work hours deemed necessary for the responsibilities assigned.
  • Ensure that client service records are maintained consistent with rules and regulations.
  • Maintain communications with all staff (e.g. admissions, support service updates and discharge plans).
  • Coordinate services with referral sources and other community resources.
  • Coordinate the RJC Sanctuary interview process including communication of monthly apartment vacancies.
  • Coordinate admissions of children to WCC including completion of admission and discharge documents.
  • Work with Sioux City Community School district and Head Start to provide education services for children not in day care.
  • Coordinate and track patient benefits (Title-10, FIP, daycare, food assistance, WIC, etc.).
  • Assist in providing and coordinating information on payment sources (daycare, food stamps, medications, etc.).
  • Assist in transportation to outside agencies such as social security, housing, public assistance, etc.
  • Collect monthly charges for patient.
  • Present workshops, lectures and training to clients, families, employees and community groups.
  • Exercise confidentiality in keeping with the Code of Conduct and with the framework of the law.
  • Deliver exceptional customer service consistently to every customer.
  • Serve as a role model for other staff, clients, and customers and demonstrate positive guest relations in representing Rosecrance.
  • Assume other related responsibilities as delegated and assigned.

Job Type : Full-time

Schedule: 8 hour shifts

  • Monday-Friday; 8:30am – 5:00pm
  • Occasional evenings as needed

Pay : Based on education, experience and credentials
